You can use the template(s) we provided in ISSAF 
(http://www.oissg.org/content/view/71/71/).
I remember that also in the SANS website (www.sans.org) there were some 
templates.

Hello all:

I need to put together an "authorization form" for clients, to allow us 
to do things like port scans, penetration testing, etc.
