I think Cisco 340 meets your requirements. but I am not sure. Is is a
windows environment or something else? You can use a domain win2k3 or win2k
server in order to use  username and passwords

As far as i know i have  not came across an ap that would do a type of  
radius authentication. Your best bet that i know of would be nocat.
