You're probably looking for a protocol rather than a client, then. XMPP (used to build the jabber network) is open, secured and extensible. Some extensions like Jingle add voice/video support. Many free clients are available, such as Pidgin, Gajim, OneTeam, empathy, sipcommunicator, gtalk, etc. Once you got a client, you may trust a service provider (the network is decentralized), use end to end encryption or set up your own server.
