Connie,

   There was a session at the Educause conference on PKI
deployment -- schools that have put elements of PKI systems
into actual production.  Re (1) below, the University of
Texas Health Sciences Center -- Houston seems to have
done a fairly substantial deployment of certificate-based
systems and, as I recall, have policies requiring the use
of encryption for sensitive information.

   I'd suggest you contact UTHSC-H directly for more information.
I think the speaker was William Weems, but you can probably
check that on the Educause site that has the conference session
descriptions.
