One good thing in their solutions  is you can blacklist certain words from
password such as <Your organization Name>.  

Check out Password Station and Password Bouncer.  

They support a plethora of operating system plugins so your users can opt
to change passwords on everything from unix hosts to databases etc. all in
one place and all at once according to their choice.  

Also has optional support for phone reset and other methods.  They have
very quick and easy demos on the website once you select a product, you
can demo right on the website.
