And in some locations, it might be institutionalized. There's certainly precedent in the physical security world; I know of several "access control systems" that all have a hard-coded master card/badge that will open any door or gate at any time. They're designed for maintenance, and theoretically they are closely guarded by the companies in question. We all know how well that works...
