IDSL will go ~30k ft, it's what I have to use at home, at $90/month :-(I had a cablemodem for a couple years and dropped it as the carrier kept changing the TOS, when I finally dropped it I found the IDSL service was almost twice as fast as the cable had slowed down to.
ISDN is a different story, it is available just about anywhere that normal phone service is available, but useually comes with per-min useage fees
