Number 3 is the most likely answer.  Americans like everything in nice
plastic packages, so #2 is probably not correct.

P.S. Extra credit question:  Why were the kids in Michigan separating out
the AC adapters from the TracFones they were buying?
  
Here's three possible reasons.:

1. An Ac Adaptor is really a transformer plus electronics. There's lots of 
copper in transformers. You need copper to make the brass that you use for 
cartridge cases. Check their homes for a factory manufacturing ammunition 
and sending it to Afghanistan.

2. Or maybe they were planning to unbundle the sale, and sell the adapters 
separately. $25 for a phone, add $10 for p&p, $10 for adaptor, $10 for 
five years insurance ...

It's a marketing idea that is commonly used.

3. They were planning to export them to a country that uses different 
shaped power points and/or with different voltages.
