uploading in meterpreter failing

fellows,

uploading in meterpreter is failing in all my sessions. I'm using 3.0
here, can anyone help me out ?

meterpreter > upload /path/to/file.exe c:/winnt/system32/file.exe
[*] uploading  : /path/to/file.exe -> c:/winnt/system32/file.exe
[-] core_channel_open: Operation failed: 3
