Try the Apache DosEvasive Maneuvers module at http://www.networkdweebs.com/stuff/security.html. On top of its basic maneuvers (returning 403's to offending IP addresses), you can also configure it to call ipchains, ipf, or some other mechanism to shut the IPs down from a network level.
