Not in America they don't. And certainly not old records.

If 60 million people each need 20kb of data on average

How many x-rays and MRI results can you store in 20kb?

My GP doesn't have any xrays or MRI scans of me.
 
And I can't imagine why you'd want to design a system that people needed
to be trained in order to use.

You'd be surprised at what people need training in. But maybe that's just
the US.

You'd design it to be like something they were already trained in.
